This service works to provide the elderly, disabled and carers with impartial advice and information.

You may be able to receive help and support with living safely and independently in your own home through the use of home adaptations and equipment. The service offers self- assessment, clinic assessment or home assessment, depending upon the issues that are identified.

Housing Support

The Housing Support service offers services for people to obtain or maintain their tenancy and to remain independent.

If you live in and own your own home, Wolverhampton Home Improvement Agency can help with grants and loans for repairs and essential maintenance to your property. Eligibility criteria apply.

Very Sheltered Housing (VSH) allows safe and secure living within a community while having care and support close by. Changing Places toilets provide extra equipment and space to enable people with profound and multiple learning disabilities, or physical disabilities such as spinal injuries, muscular dystrophy and multiple sclerosis, to use them safely and comfortably.

Contact Adult Community Services

The Care Coordination Centre (Care Co.) is for all of your Adult Community Services’ needs.

The Care Coordination Centre is open 24 hours a day, 7 days a week (including bank holidays) and supports people, their relatives and professionals to access the right services at the right time for the person who needs help.  The Team can also give health information, advice, and education.

Some of the services accessed via Care Coordination include District Nursing, Hospital at Home, Rapid Access to Social Care, Virtual Ward, Community Intermediate Care Team and Same Day Emergency Care Departments.
