Thank you for considering making a referral to the Wolverhampton Outreach Service.
We are able to accept referrals for children with additional needs that attend mainstream schools within Wolverhampton. The purpose of our support is to enable schools to adapt their practices in order to facilitate the inclusion of these children.
We accept referrals from school staff and health professionals. If you are a parent and feel your child’s school would benefit from our support, please speak to the school’s SENDCo about making a referral.
Please note that once the referral is started, you will be unable to save your progress until the referral is complete.
To complete the referral, you will need:
- Personal details of the child and their family (address, phone number, email, DOB etc.)
- School details (including contact details for your DSL)
- Evidence of the Graduated Approach being applied through the ‘Assess Plan Do Review’ cycle
- Details of the child’s academic progress
- Parental consent for engagement of the Outreach Service (including consent to observe, share information and engage 1-1 where necessary).
Once you have completed the referral form, a summary of the referral will be sent to yourself for your own records and an email notification will be sent to parents to confirm that the referral has been submitted.
